The New Jersey Water Association in cooperation with NJDEP has provided a Lead Sample Result Template and a Consumer Notice of Lead Tap Water Monitoring Results Certification Form (BSDW-54). These forms are available for your use.
This from is a requirement pursuant to 40 CFR Part 141 85(d)
The form below and a copy of the notification must be submitted to the State withing 3 months following the end of the monitoring period.
The template below can be used to inform the homeowner of their Lead Water Sample Test Results. This form as been approved by the NJDEP. Please be aware that ALL reporting results should be made in parts per billion (ppb).
